When you decide to talk to someone about how you are feeling, the list of titles can be confusing. Counsellor vs psychologist is one of the most common questions people ask, and clinical psychologists add another layer.

All three can offer helpful talking therapy. The main differences are training, how they are regulated and whether Medicare rebates apply. This guide explains each role in plain terms so you can pick a good starting point.

The short answer

If you want support with stress, relationships, grief or a life change, a counsellor or a psychologist can both be a good choice. If you have symptoms that are severe, long-lasting or complex, a psychologist or clinical psychologist is often a better starting point.

If you want to use a Medicare rebate, you will need to see a provider covered by the Better Access initiative, such as a psychologist. Counsellors are not part of that scheme.

What does a counsellor do?

A counsellor helps you talk through problems, understand yourself better and build practical ways to cope. Many counsellors work with everyday stress, relationship strain, grief, life transitions and self-esteem.

Counsellor training and regulation

Counselling is not regulated by a national government board. Healthdirect explains that counsellors and psychotherapists do not belong to a national regulating body. This means the title counsellor is not protected by law.

Many qualified counsellors hold a degree or postgraduate qualification in counselling. They often choose to register with a professional association such as the Australian Counselling Association (ACA) or the Psychotherapy and Counselling Federation of Australia (PACFA). These bodies set training standards, ongoing education and ethics codes for their members.

When you look for a counsellor, it is reasonable to ask about their qualifications and which association they belong to.

What does a psychologist do?

Psychologists are trained in how people think, feel and behave. They use structured, evidence-based therapies, such as cognitive behaviour therapy (CBT), which helps you notice and change unhelpful patterns of thinking and behaviour.

Psychologists can also assess mental health conditions and use standardised tests. This is useful when you are not sure what is going on, or when you need a report.

Psychologist training and registration

Psychologist is a protected title in Australia. The Psychology Board of Australia requires a six-year sequence of education and training before general registration. This is usually four years of accredited university study followed by two years of further training, such as a masters degree or supervised internship.

Registered psychologists must meet national standards, complete ongoing professional development and follow a code of conduct. You can check any psychologist’s registration on the public register of the Australian Health Practitioner Regulation Agency (Ahpra).

Clinical psychologists, psychiatrists and GPs

What is a clinical psychologist?

A clinical psychologist is a registered psychologist who also holds an area of practice endorsement in clinical psychology. According to the Psychology Board of Australia, endorsement requires an accredited postgraduate qualification plus a supervised registrar program.

Clinical psychologists have extra training in the assessment, diagnosis and treatment of more complex mental health conditions. That said, many registered psychologists also have a lot of experience with complex presentations. The title matters less than the person’s skills and experience with your concern.

Where do psychiatrists and GPs fit?

A psychiatrist is a medical doctor with specialist training in mental health. Psychiatrists can prescribe medicines and often see people with severe or complex conditions. Psychologists and counsellors are not doctors and cannot prescribe.

Your GP is often the first step. A GP can check for physical causes of your symptoms, discuss medication and prepare a Mental Health Treatment Plan if you are eligible.

What about neuropsychologists?

A clinical neuropsychologist is a psychologist with extra training in how the brain affects thinking, memory, attention and behaviour. Cost and Medicare rebates

Under the Better Access initiative, Medicare rebates are available for up to 10 individual and 10 group sessions each calendar year with eligible providers. These include clinical and registered psychologists, eligible social workers and eligible occupational therapists. You usually need a referral and a Mental Health Treatment Plan from your GP. Counsellors are not covered by Medicare under Better Access. Healthdirect notes that counselling may be partly covered by some private health insurance extras policies, so it is worth checking with your fund.

Some people choose a counsellor because they do not have a diagnosed mental health condition, do not want a plan on their Medicare record, or simply connect well with that person. Others prefer a psychologist so they can claim a rebate. Both are valid choices.

Questions to ask before you start

  • What are your qualifications, and who are you registered with?
  • Do you have experience with the kind of problem I am facing?
  • What approach do you use, and what will sessions look like?
  • What will it cost, and can I claim a rebate?

Feeling comfortable with your practitioner makes a real difference. After a few sessions, ask yourself whether you feel heard, respected and clear about the plan. If not, it is fine to try someone else.

Is a counsellor cheaper than a psychologist?

Not always. Fees vary between individual practitioners and practices. A psychologist may charge more per session, but if you have a Mental Health Treatment Plan, a Medicare rebate can reduce your out-of-pocket cost. Counselling has no Medicare rebate, though some private health extras policies pay part of the fee. Compare the total amount you will pay after rebates, and ask the practice for a written fee estimate.

Can a counsellor diagnose depression or anxiety?

Counsellors can help you understand your symptoms and support you to manage them. A formal diagnosis is usually made by a GP, psychiatrist or psychologist. If your counsellor thinks you may have a condition that needs assessment or medical care, they should suggest you see your GP or a psychologist. Many people see a counsellor and a GP at the same time.

Do I need a referral to see a psychologist?

No. You can book directly with a psychologist without a referral. A referral and Mental Health Treatment Plan are only needed if you want to claim a Medicare rebate under Better Access. Some other funding sources, such as workers compensation or the NDIS, have their own rules, so check before your first session if you plan to use them.

How do I check if a psychologist is registered?

You can search the public register of practitioners on the Ahpra website. It shows whether a psychologist is registered, any area of practice endorsement such as clinical psychology, and any conditions on their registration. For counsellors, you can ask which professional association they belong to and check that association’s member register.

Can I switch from a counsellor to a psychologist later?

Yes. It is common to start with one type of practitioner and change as your needs become clearer. With your consent, your counsellor can share a summary with your new psychologist so you do not have to repeat everything. If you want to use Medicare for psychology sessions, you will need to see your GP for a Mental Health Treatment Plan first.
