Do I need a cognitive assessment with my ADHD or autism assessment?

When you look at ADHD and autism assessment fees, you may see two options: an assessment on its own, or the same assessment with a cognitive assessment added. So it is a fair question to ask whether you need the extra testing.

In short, many people do not, but some people gain a lot from it. Below we explain what a cognitive assessment measures, what Australia’s guidelines say, and how to decide what fits you or your child.

اہم نکات

  • A cognitive assessment measures thinking skills such as reasoning, memory, attention and processing speed, and shows your profile of strengths and challenges.
  • Australia’s autism اور اے ڈی ایچ ڈی guidelines do not require a full cognitive assessment for every person, although the clinician must consider your general thinking ability.
  • Cognitive tests do not diagnose ADHD or autism on their own.
  • A cognitive assessment helps most when there are learning difficulties, a possible intellectual disability, an uneven profile, or a need for detailed evidence for school, work or the NDIS.
  • Your clinician can tell you whether it is likely to help in your situation before you book.

What a cognitive assessment measures

A cognitive assessment uses standardised tests to measure how you think and learn, compared with people of the same age. It usually looks at:

  • reasoning with words and with visual information
  • working memory, which is holding and using information in your head
  • processing speed, which is how quickly you take in and respond to information
  • attention, planning and organisation
  • learning and remembering new information

The result is a profile of your strengths and the areas you find harder. Similarly, the Australian Psychological Society describes this as a detailed profile of strengths and weaknesses. Our article on understanding scores and percentiles explains how results are reported.

Is a cognitive assessment required for a diagnosis?

آٹزم

Australia’s National Guideline for the assessment and diagnosis of autism lists cognitive assessment as one option when another condition, such as an intellectual disability or learning disorder, may be part of the picture. However, it does not make formal cognitive testing a requirement for everyone.

The clinician does need to consider your general thinking ability and developmental level. The diagnostic criteria ask clinicians to say whether autism comes with or without an intellectual impairment, and to tell autism apart from intellectual disability. For many teenagers and adults, school, study and work history give enough information to do this without formal testing. By contrast, for young children, or where learning has been hard, testing is more often useful.

اے ڈی ایچ ڈی

Australia’s ADHD Clinical Practice Guideline says ADHD symptoms must be assessed in the context of a person’s age, developmental level and intellectual ability. It also reports a review that found cognitive and psychoeducational tests did not make ADHD diagnosis more accurate. ADHD is diagnosed through a clinical assessment against the diagnostic criteria, using a detailed history and information from more than one person or setting.

In short, cognitive tests do not tell us whether someone has ADHD. What they can do is show how attention, memory and processing speed are affecting learning and daily life, which shapes practical recommendations.

When a cognitive assessment adds value

Situation What a cognitive assessment adds
A child or teenager is struggling at school Shows whether a learning disorder, a language difficulty or an intellectual disability may be part of the picture, and guides school supports.
An intellectual disability is possible Gives the clinician the information needed to tell autism or ADHD apart from intellectual disability, or to identify both.
You seem bright but struggle with everyday tasks Explains an uneven profile, such as strong reasoning with slow processing speed or weak working memory.
Your child may be gifted Describes strengths as well as challenges. In fact, some children are both gifted and neurodivergent.
You need evidence for the NDIS, school, university or work Gives a detailed, standardised picture of strengths and support needs to back up requests for adjustments.
There are other concerns about thinking Helps when there has been a brain injury, memory changes, long COVID or other illness that may affect cognition.

Our articles on learning disorder assessments, gifted and twice-exceptional children اور cognitive assessment and the NDIS go into these situations in more detail.

When you may not need a cognitive assessment

  • Your main question is whether you are autistic or have ADHD.
  • Your school, study and work history do not suggest a learning difficulty or intellectual disability.
  • You do not need detailed evidence for funding or adjustments right now.
  • You had a cognitive assessment recently. In that case, bring the report, as some tests should not be repeated too soon.

If you start without cognitive testing and later want it, it can usually be added as a separate assessment.

Some private health policies help with psychology costs, so check with your fund. If cost is a barrier, please talk to us. We have these conversations all the time, and it’s OK to ask.

Questions that can help you decide

  • Have learning, reading, writing or maths been hard, now or at school?
  • Do you need evidence for the NDIS, school, university or a workplace?
  • Do you notice a big gap between what you know you can do and what you get done?
  • Has there been a brain injury, illness or change in memory?
  • Have you had a cognitive assessment before, and when?

If you answered yes to any of the first four, a cognitive assessment is more likely to be worthwhile. If you are still unsure, our team can talk it through with you.

اکثر پوچھے گئے سوالات

Do I need a cognitive assessment to be diagnosed with ADHD or autism?

Not always. Australia’s autism and ADHD guidelines do not require a full cognitive assessment for everyone, but the clinician must consider your general thinking ability. A cognitive assessment helps most when there are learning difficulties, a possible intellectual disability, an uneven profile, or a need for detailed evidence.

Can a cognitive assessment diagnose ADHD?

No. Australia’s ADHD guideline reports a review that found cognitive tests did not make ADHD diagnosis more accurate. Instead, ADHD is diagnosed through a clinical assessment against the diagnostic criteria. However, cognitive tests can show how attention and memory affect daily life.

What does a cognitive assessment measure?

It measures thinking skills such as reasoning, working memory, processing speed, attention, planning and learning, compared with people of the same age. The result is a profile of strengths and challenges.

Should my child have a cognitive assessment with their autism or ADHD assessment?

It is often worthwhile for children who are finding learning hard, where a learning disorder or intellectual disability is possible, or where the school needs detailed information to plan support. Your clinician can advise before you book.

How much does a cognitive assessment add to an ADHD or autism assessment at Invictus Health?

An ADHD assessment is $1,900 and an autism assessment is $2,100. With a cognitive assessment added, either one then costs from $3,300 to $3,600. A combined ADHD and autism assessment is $2,700, or $3,850 with a cognitive assessment.
