It can help you understand what is getting in the way, reduce anxiety and shame, and find practical ways forward.

It is for individuals and couples of any age, gender, sexuality or relationship style. You do not need a partner to attend.

What sex therapy is not

Sex therapy is talk-based. There is no physical contact, no nudity and no sexual activity in sessions. Your therapist will never ask you to undress or touch you. If you are given exercises to try, you do them privately at home, at your own pace, and only if you choose to.

Who provides it?

Sex therapy is usually provided by psychologists, counsellors or doctors who have extra training in sexual health and sexology. Sex therapist is not a protected title in Australia, so it is reasonable to ask about a practitioner’s qualifications, registration or professional membership, and their experience with your concern.

This article is general information. Please speak with a qualified health professional about your own situation.

What can sex therapy help with?

People seek sex therapy for many reasons, including:

low sexual desire, or a difference in desire between partners

difficulty getting or keeping an erection

ejaculating sooner or later than you would like

difficulty reaching orgasm

pain during sex, or vaginismus (involuntary tightening of the muscles around the vagina)

anxiety, shame or guilt about sex

changes after childbirth, menopause, illness, surgery or cancer treatment

the effects of medicines on sex, such as some antidepressants

sex and intimacy after sexual trauma

questions about sexual identity, fantasies or relationship agreements

feeling that sexual behaviour is out of your control

Healthdirect notes that nearly 1 in 2 Australian men have experienced a sexual difficulty. Sexual concerns are a normal part of human experience, not a personal failing.

Why a medical check can help

Sexual difficulties often have more than one cause. Jean Hailes for Women’s Health describes how physical health, pelvic floor problems, medicines, hormones, stress, mood, body image, relationship issues and past trauma can all affect sex.

Some physical causes need medical care. For example, Healthdirect explains that erectile dysfunction can be an early sign of heart disease, diabetes, high blood pressure or high cholesterol. Pain during sex should also be checked by a doctor.

That is why a sex therapist may suggest you see your GP, or work alongside a gynaecologist, urologist or pelvic health physiotherapist. Treating the body and the mind together is often the most helpful approach.

What happens in a session

Preparing for your first session

It can help to jot down a few notes beforehand: what the concern is, when it started, what you have already tried and what you hope will change. Bring a list of any medicines you take. If you feel nervous, you can say so at the start. Therapists are used to people feeling awkward, and it usually eases quickly.

Your first session

Your therapist will explain confidentiality and how they work. They will then ask about the concern that brought you in, when it started and what makes it better or worse. They may ask about your general health, medicines, relationships, upbringing and beliefs about sex.

You choose how much to share. A good therapist uses plain, non-judgemental language and moves at a pace that feels comfortable. Many people find that simply saying things out loud to someone who is not shocked brings relief.

Later sessions

Depending on your goals, sessions might include:

Education: accurate information about bodies, arousal and desire, which can correct common myths.

Working with thoughts and feelings: strategies from cognitive behaviour therapy (CBT) and mindfulness to reduce performance anxiety and self-criticism.

Communication: practising how to talk with a partner about needs, limits and preferences.

Home exercises: private, step-by-step activities, such as structured touch exercises for couples, that take pressure off and rebuild comfort.

Common beliefs that get explored

Many sexual worries are made worse by unhelpful beliefs. A therapist may gently question ideas such as:

everyone else is having more, or better, sex

good sex should always be spontaneous

sex only counts if it involves intercourse

a difference in desire means something is wrong with the relationship

Loosening these ideas often reduces pressure, and less pressure can make it easier to enjoy intimacy again.

Couples and individuals

If you have a partner, you can attend together, alone or a mix of both. Sexual concerns often affect both people, and working as a team can reduce blame and pressure.

How long does it take?

This varies. Some people find a few sessions of information and reassurance are enough. Others, particularly where there is trauma, pain or a long-standing pattern, benefit from longer support. Your therapist should check in regularly about how things are going and whether the plan still fits.

Does sex therapy work?

A 2013 systematic review and meta-analysis looked at 20 randomised controlled trials comparing psychological treatments with a waitlist. It found that psychological treatments reduced the severity of sexual difficulties and improved sexual satisfaction.

The evidence was strongest for low sexual desire and orgasm difficulties in women. For some other concerns, fewer high-quality studies exist, and more research is needed. In practice, many people benefit from combining therapy with medical treatment where that is relevant.

Sex therapy after sexual trauma

If you have experienced sexual abuse or assault, sex and intimacy can bring up fear, numbness or flashbacks. A trauma-informed therapist will go slowly, focus on your sense of safety and control, and never push you to share more than you want to.

For support with sexual, domestic or family violence, call 1800RESPECT on 1800 737 732, 24 hours a day.
