It usually involves two broad approaches:<\/p>\n<ul>\n<li><strong>Restoring skills:<\/strong> practising specific skills so they work better, often in the real situations where they are needed.<\/li>\n<li><strong>Compensating:<\/strong> using tools, routines and strategies to work around difficulties that remain.<\/li>\n<\/ul>\n<p>Good rehabilitation is built around your own goals. For one person that might be returning to work. For another it might be cooking a family meal, managing medicines or catching the train alone.<\/p>\n<h3>Setting clear goals<\/h3>\n<p>Clear, specific goals make it easier to choose strategies and see progress. For example, instead of a general goal like improving memory, a person might aim to take their evening medicines on time every day for a month using a phone alarm and a pill organiser. Small, concrete goals build confidence and show what is working.<\/p>\n<p><a href=\"https:\/\/www.healthdirect.gov.au\/rehabilitation-after-illness-or-injury\">healthdirect<\/a> notes that rehabilitation can involve a team, including rehabilitation doctors, psychologists, occupational therapists, speech therapists, physiotherapists and social workers. Your GP and treating team can help with these.<\/p>\n<h2>Strategies for memory<\/h2>\n<p>The INCOG 2.0 guideline on <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/36594861\/\">memory<\/a> supports both external aids and internal strategies.<\/p>\n<h3>External aids<\/h3>\n<p>External aids are tools outside your head that hold information for you. The guideline recommends them as the main strategy for severe memory problems, and prefers electronic reminders such as smartphones over older systems. Useful examples include:<\/p>\n<ul>\n<li>phone calendar alerts for appointments, medicines and tasks<\/li>\n<li>a single diary or notebook that goes everywhere with you<\/li>\n<li>a whiteboard or noticeboard in a central place at home<\/li>\n<li>checklists for routines, such as leaving the house<\/li>\n<li>voice notes to capture ideas or instructions<\/li>\n<\/ul>\n<p><a href=\"https:\/\/synapse.org.au\/fact-sheet\/memory-problems-and-tips\/\">Synapse<\/a> suggests other simple changes, such as keeping a notepad by the phone, labelling cupboards, and having set places for important items like keys and wallet.<\/p>\n<h3>Routines and habits<\/h3>\n<p>Routines reduce how much you need to remember. Doing the same things in the same order, at the same times, means tasks become automatic over time. For example, checking the calendar every morning at breakfast, or putting keys in the same bowl every time you come home, takes pressure off memory.<\/p>\n<h3>Internal strategies<\/h3>\n<p>Internal strategies are mental techniques, such as linking new information to a picture, repeating it at spaced intervals, or grouping items into categories. They tend to suit people with milder memory difficulties.<\/p>\n<h3>Learning without errors<\/h3>\n<p>The memory guideline recommends teaching strategies in a way that limits mistakes while learning. People with memory problems can accidentally learn their errors. A therapist might guide you step by step and fade support gradually, rather than asking you to guess.<\/p>\n<h2>Strategies for attention and concentration<\/h2>\n<p>Attention problems can make it hard to focus, filter out background noise or do two things at once. The INCOG guideline recommends metacognitive strategy training, which means learning to notice and manage your own thinking during everyday activities, for people with mild to moderate attention problems.<\/p>\n<p>It also notes that practising computer-based attention tasks on their own is not recommended, because gains do not seem to carry over to daily life. Practising the real task, such as following a conversation in a noisy room, is more likely to help with that task. Practical ideas include:<\/p>\n<ul>\n<li>do one thing at a time<\/li>\n<li>reduce noise and clutter, and silence notifications while working<\/li>\n<li>schedule demanding tasks for when you are most alert<\/li>\n<li>break reading or work into short blocks with breaks<\/li>\n<li>ask people to speak one at a time, and to slow down<\/li>\n<\/ul>\n<h2>Strategies for planning and problem solving<\/h2>\n<p>Executive functions are the thinking skills that help you plan, organise, start tasks, stay on track and adjust when things change. The INCOG 2.0 guideline on <a href=\"https:\/\/pubmed.ncbi.nlm.nih.gov\/36594859\/\">executive functions<\/a> supports metacognitive strategy instruction, such as goal management training. This approach teaches a person to pause, state the goal, break it into steps and check progress along the way.<\/p>\n<p>The guideline also supports ways to improve self-awareness, such as structured feedback and reviewing video of your own performance with a therapist. Practical tools include:<\/p>\n<ul>\n<li>writing a plan before starting a task, with each step listed<\/li>\n<li>using a simple routine: stop, plan, do, check<\/li>\n<li>setting alarms to prompt a check-in partway through a task<\/li>\n<li>using templates for things you do often, like shopping lists or meal plans<\/li>\n<li>talking through decisions with a trusted person before acting<\/li>\n<\/ul>\n<h2>Managing fatigue<\/h2>\n<p>Fatigue affects every other thinking skill. Synapse describes <a href=\"https:\/\/synapse.org.au\/fact-sheet\/managing-fatigue\/\">fatigue after brain injury<\/a> as common and highly disabling, and suggests:<\/p>\n<ul>\n<li>building regular rest breaks into your day<\/li>\n<li>learning your limits and stopping before you reach them<\/li>\n<li>breaking tasks into smaller parts<\/li>\n<li>organising your routine around the times you have most energy<\/li>\n<li>noticing triggers, such as noisy places or long conversations<\/li>\n<li>keeping good sleep habits, staying active and limiting alcohol<\/li>\n<\/ul>\n<p>For example, if reading becomes tiring after 30 minutes, you might read for 20 minutes and then take a break.<\/p>\n<h3>Involving family and friends<\/h3>\n<p>Strategies work better when the people around you understand them. Family members can help by using the shared calendar, giving information one step at a time, and prompting rather than taking over. It helps to agree together on what support is useful.
