They described potentially morally injurious events as “perpetrating, failing to prevent, bearing witness to, or learning about acts that transgress deeply held moral beliefs and expectations”.

Phoenix Australia, the national centre for posttraumatic mental health, defines moral injury as “the psychological, social and spiritual impact of events involving betrayal or transgression of one’s own deeply held moral beliefs and values occurring in high stakes situations”. This definition appears in its guide to moral injury for health care workers.

Put simply, moral injury can arise when:

you do something, or fail to do something, that goes against your values

you witness others doing something you believe is seriously wrong

you feel betrayed by people in authority, such as leaders or the organisation

Moral stress, moral distress and moral injury

Phoenix Australia describes a continuum. Moral stressors are common in many jobs. Most cause short term frustration. Some cause moral distress, a more lasting unease. At the far end, rare and severe events can lead to moral injury, which has an enduring effect on how people see themselves and the world.

Examples of moral stressors at work might include:

a health worker unable to give the care they know a patient needs, because of staffing, beds or equipment

a child protection or disability worker who must follow a policy they believe will cause harm

a police officer or emergency worker who could not prevent a death

a manager directed to carry out decisions they see as unfair or dishonest

an employee who raised a serious concern and was ignored, blamed or punished

Not everyone exposed to these situations develops moral injury. Personal history, support, the severity of the event and how the organisation responds all make a difference.

Signs of moral injury

The Phoenix Australia guide describes effects across several areas of life:

Emotions: guilt, shame, anger, sadness, anxiety and disgust

How people see themselves: harsh self-criticism, feeling damaged or unworthy

Relationships: loss of trust in others, including the organisation, avoidance and reduced empathy

Meaning and purpose: questioning beliefs, feeling that work no longer makes sense

Behaviour: withdrawing from others, drinking or using other substances more, or other self-destructive behaviour

Moral injury is not a diagnosis in the DSM-5 (the main diagnostic manual for mental health conditions). However, Phoenix Australia notes that it is a risk factor for PTSD, depression, anxiety, insomnia and suicidal thoughts.

If you or someone you work with is in danger, call 000. For 24-hour crisis support call Lifeline on 13 11 14.

How moral injury differs from burnout and PTSD

Core experience

Common feelings

Burnout

Long term work stress that has not been managed well

Exhaustion, cynicism, reduced effectiveness

PTSD

Exposure to a traumatic event involving threat or danger

Fear, intrusive memories, feeling on edge, avoidance

Moral injury

Events that violate deeply held values, or betrayal by trusted people

Guilt, shame, anger, loss of trust and meaning

These can overlap. Phoenix Australia’s guide suggests some of what is called burnout in health care may be better understood as moral distress or injury. That is, the problem may be the gap between knowing what is right and being unable to do it, rather than a lack of personal resilience.

Research supports the idea that moral injury is distinct. An international project involving Phoenix Australia developed the Moral Injury Outcome Scale, which measures two main dimensions: shame-related outcomes and trust violation outcomes.

How organisations can respond

Moral injury is not only an individual problem. The way work is organised, and how leaders behave, can raise or lower the risk. Moral stressors often overlap with psychosocial hazards that employers must manage, such as exposure to traumatic events, low job control, poor support and poor organisational justice.

At the organisational level

Drawing on Phoenix Australia’s guidance, organisations can:

openly acknowledge that some roles involve moral stressors

develop clear, fair policies for ethically difficult decisions, such as how scarce resources are allocated

where possible, move the hardest ethical decisions away from individual frontline workers

rotate staff between higher and lower stress roles

make psychological and peer support easy to access, without stigma

make sure people who raise concerns are heard and protected

At the team level

make time to discuss ethical challenges openly, not only clinical or technical ones

help staff make sense of difficult decisions and the constraints they faced

check in regularly and offer support early

acknowledge moral dilemmas honestly, and recognise good work

model healthy coping and taking breaks

After a difficult event

Check in with the people involved soon after, and again in the following weeks. Moral injury can build over time.

Offer practical support first, such as time off the floor, a quieter shift or help with paperwork.

Give people accurate information about what happened and why decisions were made.

Avoid forcing anyone to retell the event in detail in a group setting. Let people choose how and when they talk.

Look at what the organisation can learn and change, so the same moral bind is less likely to happen again.

Leaders also have a role in preventing betrayal. Being honest about constraints, explaining decisions and following through on promises all help protect trust.

Supporting someone who may be affected

If a colleague seems weighed down by guilt, shame or anger after a difficult event, listen without judging and avoid rushing to reassure them that they did nothing wrong. Encourage them to speak with their GP, a psychologist or their employee assistance program. Evidence-based trauma treatments can be adapted when moral injury is part of the picture. If they mention thoughts of suicide or self-harm, ask about it directly and help them connect with crisis support. Asking about suicide does not put the idea in someone’s head.
