Yet many adults say emotions are one of the hardest parts. They may feel frustration, hurt or excitement very quickly and very strongly, and find it hard to calm down.

Clinicians call this emotion dysregulation. It means difficulty managing the strength and length of emotional reactions. A 2020 meta-analysis combined 13 studies with 2,535 participants. It found that adults with ADHD reported much higher levels of emotion dysregulation than adults without ADHD. Emotional lability, meaning moods that shift quickly, showed the strongest difference (Beheshti and colleagues, 2020).

Emotional intensity is not all hard. Many people with ADHD also feel joy, enthusiasm, care and a strong sense of fairness very deeply. Support aims to give you more choice about how you respond, while keeping the depth of feeling that many people value.

The Australian ADHD guideline recommends that education for people with ADHD include information about regulating emotions.

What rejection sensitivity is

Rejection sensitivity is a tendency to expect rejection, notice it quickly and react to it strongly. Everyone has some level of rejection sensitivity. For some people it is much stronger.

Research suggests rejection sensitivity is higher in people with ADHD symptoms. A German study of 1,235 young people aged 10 to 19 found that those with ADHD symptoms reported higher anxious and angry rejection sensitivity than their peers (Bondü and Esser, 2015).

What about RSD?

Rejection sensitive dysphoria, or RSD, is a term that has spread widely online. It describes intense emotional pain in response to real or perceived rejection or criticism. In a study of young adults with ADHD, participants described their experiences of emotional dysregulation, including RSD, and said the diagnostic criteria did not fully capture them (Ginapp and colleagues, 2023).

RSD is not a formal diagnosis, and emotion dysregulation is not part of the formal diagnostic criteria for ADHD. Researchers describe rejection sensitivity in ADHD as relatively unexplored. A small 2026 study of five university students with ADHD found three themes in how they experienced perceived rejection: withdrawal, masking their feelings, and strong bodily sensations (Rowney-Smith and colleagues, 2026).

Because research is still limited, it helps to treat RSD as a description of experience rather than a separate condition.

Whatever words you use, the feelings are real and deserve support.

What it can look like

feeling deeply hurt or ashamed after small criticism

replaying a conversation for hours, looking for signs someone is upset with you

a sudden flash of anger when you feel judged

people-pleasing or saying yes to avoid disappointing anyone

not applying for jobs, sharing work or trying new things, in case of rejection

pulling away from friends after a perceived slight

hiding your feelings so others will not see how hurt you are

Social experiences matter too. Many adults with ADHD describe growing up with a lot of correction and criticism. It makes sense that criticism can feel loaded later in life.

Skills that can help

The skills below draw on ideas from dialectical behaviour therapy (DBT) and cognitive behavioural therapy (CBT). You can practise them on your own, with a coach or with a psychologist.

Name the emotion

Put a word to what you feel, such as hurt, embarrassed or angry. Naming an emotion can take a little of the heat out of it and help you choose what to do next.

Check the facts

Ask yourself: what actually happened? What am I assuming? What else could explain it? A short reply to your message might mean the person was busy, not upset with you.

Pause before responding

Strong emotions usually peak and then ease. If you can, wait before you reply. Write the message you want to send, then save it as a draft and look at it later.

Calm your body first

Slow breathing, a walk or a change of room can help your body settle. It is often easier to think clearly once your body is calmer.

Ask for clarity

If feedback is vague, ask a simple question, such as can you tell me what you would like me to change? Clear information often feels less threatening than guessing.

Look after the basics

Tiredness, hunger and stress make emotions harder to manage for everyone. Sleep, regular meals and taking any prescribed medication as directed can all help.

Plan ahead for known triggers

Some situations are predictable, such as performance reviews, feedback on an assignment or a hard family conversation. Plan what you will do before, during and after. For example, you might ask for the main points in writing, take a short walk afterwards and wait a day before you respond.

Make a card for hard moments

Strong emotions make it hard to remember what helps. Write three or four steps on a card or in your phone, such as breathe out slowly, name the feeling, do not reply yet and text a friend. Keep it somewhere easy to find.

Be kind to yourself

Try talking to yourself the way you would talk to a friend who felt this way. Self-criticism tends to make the hurt last longer.

What the research says about therapy

Psychological therapies can help adults with ADHD. A 2025 meta-analysis of 8 randomised controlled trials, with 850 adults, found that DBT moderately reduced ADHD symptoms and improved quality of life compared with control conditions (Zhang and colleagues, 2025).

Therapy is provided by qualified clinicians, such as psychologists. It may suit you if strong emotions are causing ongoing problems in relationships or work, or if you also have anxiety, depression or past trauma.

Please seek help if you have thoughts of harming yourself, if you are using alcohol or other drugs to cope, or if you feel hopeless.
